Freedom of Information Act
Community High School District 155 strives for transparency in its activities. In addition to information available through this website, in newsletters, and available at public meetings, members of the public may request documents through the Illinois Freedom of Information.
Jeff Puma, director of communications for District 155, serves as the district's Freedom of Information Act Officer. All requests should be submitted to Mr. Puma. Requests can be made through email, in-person at the District 155 Center for Education, fax or by U.S. mail. Contact information for submitting a request is below:

For the requesters convenience, District 155 provides a form that can be filled out when making a request in person. This form also can be downloaded here in PDF format and submitted via fax, U.S. mail, or as an attachment to email.
Details of Freedom of Information Act Requests submitted to the district and available for public inspection are listed each month in the board of education agenda. This information is posted to the district website no later than 48 hours prior to each month's meeting, and an archive is maintained on the site.
